Forms
We have included some forms which may be of interest to you. First is the "Chief's Authorization Form". This form is currently used to pre-register for classes hosted by the Chenango County Bureau of Fire. We encourage all students who are interested in attending any of our classes utilize this form. We take pre-registration seriously. As we all know, there is a minimum and maximum number of students who are allowed to attend these classes. We use these pre-registrations to be sure every student is accounted for and meets the necessary requirements. Also we use these forms to determine if a class will have the necessary number of students or if another class will need to be scheduled due to the enrollement. A summary of our policy is as follows:
1) Pre-registration is required for EVERY Course.
2) Preference will be given to students who have pre-registered over those who have not. If the course is full, all students that do not pre-register will be unable to attend the course.
3) If a student pre-registers and does not attend the course, or notify the Deputy Coordinator - Supervisor of Fire Training and/or the Instructor of the absence, they may not be allowed to attend future classes for a period not to exceed one year.
4) Students from a Department housed in Chenango County will receive preference over students attending from outside Chenango County. If space is available, then all pre-registered students will be allowed to attend.
5) All prerequisites will be verified. If a student does not have the required prerequisites, they will not be eligible to attend the class until such prerequisites are established.
6) If required, a proof of recent SCBA fit testing and physical eligibility will be verified. If a student does not provide proof of proper SCBA fit testing or meets the physical requirements of the class, within the allowed timeframe, they will be ineligible to attend the course.
Pre-Registration Form (new)
Also included is our "Use & Maintenance Form" for our Fire Training Center. We ask every agency that uses the Training Center to complete this form. It helps our staff to address any problems or concerns which may occur during your training at our Training Facility. Simply, complete and "Send the Form" to us and we will utilize this form to make necessary improvements.
The Perma Quick Fax Report and the VF2 Insurance form are to be completed each and every time a firefighter is injured during training or an actual event. Please complete these forms and fax them directly to our Insurance Administrator or you may fax them to the Fire Coordinators office.
